On Track School requires a 30 day notice for all students who wish to withdraw from the program. By completing this form, you are requesting that your student be withdrawn from On Track School and you understand that if you have a payment due within the 30 day notice period that payment will be processed along with any additional fees due at that time. Please note, high school students who wish to drop prior to the end of the semester will not be provided with completed credits unless all work for the semester has been completed and requirements for the full semester meet.
